Submit this form, print the resulting page, and send the form including Card No and Expiration Date by Fax to the bursar.
US Bank Check Checks must be drawn on a US bank or US branch of a non-US bank, payable to EG/ACM Workshop on Graphics Hardware '99.
Bank Transfer Please make sure that the beneficiary receives the indicated amount in spite of the bank fees for the transaction!
Cash Pay at the workshop registration desk.

This registration process is preliminary and the registration will only be confirmed (by email) after payment has been completed. If you have completed an on-line registration but have not sent your payment, you are not registered. If the payment does not arrive within 3 weeks after submission of this registration form, your registration is not effective. Please include a paper copy of your registration form with the payment of the registration fees.

No-shows or cancellations after July 1, 1999, are liable for full registration fee. Cancellations must be made in writing. Substitutions are welcome, but must be submitted in writing prior to start of conference.
